Dear Parents:
Please take note of the medical requirements for all Kindergarten students.
As required by the Illinois School Code and the Illinois Department of Public Health, please know that ALL students entering kindergarten are required to complete and submit the following health requirements to the school nurse.
ALL students entering Kindergarten are required to present:
● Proof of a school physical, completed by a physician, with health history section completed and signed by the parent (the parents signature date should be the same as the physician's date.
● Proof of up to date immunizations ( if you child is behind on their immunizations, a catch schedule must be submitted to the nurse. To avoid being excluded, this schedule must be followed.
● Proof of a vision exam completed by an Optometrist or Ophthalmologist.
● Proof of a dental examination: if needed, Orland Park Dental Services will be providing dental services at all of our elementary schools during the month of October. Please allow each school to assist you with meeting this requirement by signing your child up for a free cleaning and examination. Proof of examination will be sent home with each student. Por favor, tomen nota de los requisitos médicos para todos los estudiantes de Kindergarten. Es Requerido por el Código Escolar de Illinois y el Departamento de Salud Pública de Illinois que todos los estudiantes que ingresan a Kindergarten deben cumplir y presentar los siguientes requisitos de salud a la enfermera escolar. Todos los estudiantes que ingresan a Kindergarten deben presentar:
● Prueba de un examen físico escolar, realizado por un médico, con la sección de historial médico completada y firmada por el padre o tutor (la fecha de la firma del padre debe coincidir con la fecha del médico).
● Prueba de vacunas actualizadas (si su hijo está atrasado con las vacunas, debe presentar un plan de vacunación a la enfermera. Para evitar la exclusión, este plan debe de seguirse ).
● Prueba de un examen de la vista realizado por undoctor de la vista.
● Prueba de un examen dental: si lo necesita, Orland Park Dental Services brindará servicios dentales en todas nuestras escuelas primarias durante el mes de octubre. Permita que cada escuela le ayude a cumplir con este requisito inscribiendo a su hijo para una limpieza y examen dental gratis. La prueba del examen será enviada a casa con cada estudiante.
